To convey the medical care philosophy of “Human-Centered”, Tungs’ Taichung Metro Harbor Hospital established “Diabetes Care Center” on March 1, 2017. The medical teams of the center provide services to patients with “diabetic related diseases” and their families. The teams integrate related medical resources and expertise and construct complete medical service systems to provide the most satisfying and professional holistic health care to patients and their families.
“Diabetes Care Center” is consisted of teams from Metabolism, Internal Medicine, Emergency Center, ICU of Internal Medicine, Metabolism Ward, Nursing Department, Pharmacy, Nutrition Therapy, Clinical Pathology, Social Work, Community Services, IT and Quality Control Center that customize care plans based on each patient’s conditions to provide holistic health care. The medical teams not only treat acute and chronic diseases, but also improve and reduce the risk factors and lower the incidence rate of acute and chronic complications. The medical teams offer hygiene education on health care and nutrition of diabetic related issues, such as hyperglycemia, hypoglycemia, hyperlipidemia, hypertension, medication, insulin injection, exercise, feet care, obesity, wound care, and smoking, to maintain good living quality. When consented, the medical teams may also provide phone and other related consultation services.
Glycated hemoglobin (HbA1C) is used to remind patients of their ideal goal of glycated hemoglobin control. HbA1C shows the average sugar level in the past three months. Half of the HbA1C is from the sugar level in the most recent month, while the other half of the HbA1C is contributed by the average sugar level from the earlier two to three months. According to the large-scale clinical investigation, complications may be effectively reduced when HbA1C is under “7”. Therefore, let’s work together to reach the goal of ≤ “7”
